When adding a new physical self-check record, the following information is recorded:

• Year of Implementation: Select from available years (e.g., option1, option2, option3, option4).
• Activity/Sub-Model: Specify the activity or sub-model (e.g., plantation, check dam construction).
• Cost Norm Code: Code for cost norms applied to the activity.
• Area: Area covered by the activity.
• No. of Trees: Number of trees planted.
• No. of Bamboo Culms: Number of bamboo culms planted.
• Length Implemented: Length of the implemented structure or activity.
• No. of Check Dams: Number of check dams constructed.
• Dimensions of Check Dams: Dimensions of the constructed check dams.
• Physical Variation: Any physical discrepancies observed.
• Financial Variation: Any financial discrepancies observed.
• % Age Acceptance: Percentage of activity acceptance based on verification.
• Cost Accepted: Accepted cost after verification.
• Recommended for Reimbursement: Amount recommended for reimbursement.
